Creating abuse reporting form

Hey guys,

How are you guys doing today ?

I would like to create some abuse reporting form, I know how it should work but I have not completely figured out how.
I would like to create a url like this

https://mydomain.com/abuse/abuse.php?broadcast_id=[CAMPAIGN_UID]&subscriber_id=[SUBSCRIBER_UID]

Then the abuse.php should be able to locate the campaign and user responsible for this complaint
